"Objective: Numerous studies has shown that regular physical exercise can reduce musculoskeletal pain, but the optimal setting to achieve high adherence and effectiveness remains unknown. This study investigated the effect of workplace versus home-based physical exercise on musculoskeletal pain among healthcare workers.Methods: The randomized controlled trial (RCT) comprised 200 female healthcare workers from 18 departments at 3 hospitals. Participants were randomly allocated at the cluster level to ten weeks of: (i) workplace physical exercise (WORK) performed during working hours for 5×10 minutes per week and up to 5 group-based coaching sessions on motivation for regular physical exercise, or (ii) home-based physical exercise (HOME) performed during leisure time for 5×10 minutes per week. Both groups received ergonomic counseling on patient handling and use of lifting aides. Average pain intensity (0–10 scale) in the low back and neck/shoulder was the primary outcome.Results: Per week, 2.2 (SD 1.1) and 1.0 (SD 1.2) training sessions were performed in WORK and HOME groups, respectively. Pain intensity, back muscle strength and use of analgesics improved more following WORK than HOME (P<0.05). Between-group differences at follow-up (WORK versus HOME) was -0.7 points for pain intensity [95% confidence interval (95% CI) -1.0– -0.3], 5.5 Nm for back muscle strength (95% CI 2.0–9.0), and -0.4 days per week for use of analgesics (95% CI -0.7– -0.2). The effect size for between-group differences in pain intensity was small (Cohen's d=0.31).Conclusions: Workplace physical exercise is more effective than home-based exercise in reducing musculoskeletal pain, increasing muscle strength and reducing the use of analgesics among healthcare workers."

"Objectives This prospective cohort study investigates work-related risk factors for occupational back injury among healthcare workers. Methods The study comprised 5017 female healthcare workers in eldercare from 36 municipalities in Denmark who responded to a baseline and follow-up questionnaire in 2005 and 2006, respectively. Using logistic regression, the odds for occupational back injury (ie, sudden onset episodes) in 2006 from patient transfers in 2005 was modeled. Results In the total study population, 3.9% experienced back injury during follow-up, of which 0.5% were recurrent events. When adjusting for lifestyle (body mass index, leisure-time physical activity, smoking), work-related characteristics (seniority and perceived influence at work), and history of back pain and injury, daily patient transfers increased the risk for back injury (trend, P=0.03): odds ratio (OR) 1.75 [95% confidence interval (95% CI) 1.05–2.93] for 1–2 transfers per day, OR 1.81 (95% CI 1.14–2.85) for 3–10 transfers per day, and OR 1.56 (95% CI 0.96–2.54) for >10 transfers per day, referencing those with <1 patient transfer on average per day. The population attributable fraction of daily patient transfer for back injury was estimated to be 36%. Among those with daily patient transfer (N=3820), using an assistive device decreased the risk for back injury for “often” and “very often” use [OR 0.59 (95% CI 0.36–0.98) and OR 0.62 (95% CI 0.38–1.00), respectively] referencing those who “seldom” use assistive devices. Conclusion Daily patient transfer was associated with increased risk for back injury among healthcare workers. Persistent use of an assistive device was associated with reduced risk for back injury among healthcare workers with daily patient transfers."

"Objective The aim of this study was to investigate the risk of persistent low-back pain (LBP) based on the number of patient-handling activities among female healthcare workers with no LBP and those with sub-chronic LBP at baseline.Method Female healthcare workers in the eldercare services answered a questionnaire about the number of patient-handling activities (<1, 1–2, 3–10, >10 per day) and days with LBP in 2005. We prospectively investigated the odds ratio (OR) for developing persistent (>30 days in the past 12 months) LBP in 2006 from the frequency of patient-handling activities using multi-adjusted logistic regression analysis among female healthcare workers without LBP (0 days in the past 12 months) (N=1544) and with sub-chronic LBP (1–30 days in the past 12 months) (N=2294) in 2005. Results Among female healthcare workers with sub-chronic LBP at baseline, the multi-adjusted OR for developing persistent LBP was 1.04 [95% confidence interval (95% CI) 0.71–1.52] for those with 1–2 activities, 1.29 (95% CI 0.91–1.83) for those with 3–10 activities, and 1.61 (95% CI 1.07–2.42) for those with >10 patient-handling activities per day (P=0.01 for trend), referencing those not performing patient-handling activities. Among female healthcare workers without LBP at baseline, we did not find an increased risk for developing persistent LBP within one year resulting from performing several patient-handling activities. Conclusion Preventive initiatives for persistent LBP may aim to keep the number of patient-handling activities below ten per day among healthcare workers with sub-chronic LBP."

"Construction work is physically demanding and often associated with bodily pain. This article presents a study of construction workers' practices of using and relating to their bodies at work through an agential realist framework for analysing the (re)configuration of the workers' embodied subjectivity. The analysis draws on interviews with 32 Danish construction workers as well as brief observations. The article shows how ‘trading health for money' becomes a mode for maintaining positive social, occupational and masculine identity among construction workers. Furthermore, it shows how the agency of the body is overruled by the intra-acting agencies of productivity, collegiality, job security and masculine working-class identity. Finally, it shows an instability in this configuration of masculine working-class identity that leaves room for a focus on the body. "

"Objective
To determine the prospective association between retrospectively assessed physical work environment during working life and prospectively assessed sickness absence and labour market exit among older workers.
Methods
Using Cox regression analyses we estimated the 4-year to 6-year prospective risk of register-based long-term sickness absence (LTSA), disability pension, early retirement and unemployment from exposure to different physical work environmental factors during working life among 5076 older workers (age 49–63 at baseline) from the Copenhagen Aging and Midlife Biobank cohort.
Results
Very hard physical work throughout working life was a risk factor for LTSA (HR 1.66,95% CI 1.32 to 2.07), disability pension (HR 2.21,95% CI 1.04 to 4.72) and early retirement (HR 1.57,95% CI 1.13 to 2.17). Both short-term (<10 years) and long-term (≥20 years) exposures to lifting or carrying of heavy burdens predicted the risk of LTSA (HRs 1.49–1.56) and disability pension (HRs 2.26–3.29). In contrast, exposure to dust was associated with LTSA and disability pension only following 20 or more exposure years.
Conclusions
Retrospectively assessed hard physical work during working life and exposure to several factors in the physical work environment, especially heavy lifting, were important for labour market exit and sickness absence. This study underscores the importance of reducing physical work exposures throughout the working life course for preventing sickness absence and premature exit from the labour market."

"Background
Most studies on the psychosocial working environment have focused on evaluating the isolated effect of individual psychosocial work factors or looked at effects through a lens of theories such as job strain or effort–reward imbalance. However, to fathom the intricate nature of workers' experience of occupational strain, there is a need to investigate the combined and cumulative effects of multiple exposures to psychosocial work factors on workers' health.
Methods
In this prospective cohort study, we created an additive index (range 0–4) on number of baseline exposures to quantitative demands, emotional demands, role conflicts, and workplace bullying. Via logistic regression and Cox regression, we estimated the association between the additive index of psychosocial work factors and depressive disorder and long-term sickness absence (LTSA). We assessed the onset of depressive disorder using the Major Depression Inventory at 6-month follow-up and the onset of LTSA using a national register during 12-month follow-up.
Results
For onset of depressive disorder, high exposure to any one [odds ratio (OR) 2.98], two (OR 3.14), three (OR 6.44) and all four (OR 9.62) adverse psychosocial work factors predicted a statistically significant increased risk. For onset of LTSA, high exposure to any one [hazard ratio (HR) 1.13], two (HR 1.67), three (HR 2.31) and all four (HR 4.04) psychosocial work factors predicted an increased risk. The two latter associations were statistically significant. Trend tests indicated an exposure–response relationship for both outcomes.
Conclusions
Workers reporting exposure to multiple adverse psychosocial work factors had a higher risk of developing depressive disorder and LTSA."
